Middle School Admissions Process 2021-2022

The Middle School Admissions Process has begun!

Please know that this process consists of several steps in choosing your child's Middle School.   All 5th grade students are to complete an online application, regardless of what school you would like to be considered for. When the application becomes available, families will receive a letter from the Department of Education with instructions for the first step. We will host middle school admissions process workshops here at P.S. 128. The dates and times are to be determined.  Please click on the link below for instructions for what steps to take now.

Middle School Open Houses

Below please find information for I.S. 96 Seth Low and Kingsborough Early College Secondary School Open Houses.  I strongly encourage all to attend these open house so that you may be informed of what each school offers their students.  This is a great tool to help you in choosing a Middle School for your child.

Sheepshead Bay Library Celebrating 125 Years of the Brooklyn Public Library

The Sheepshead Bay Library, located at 2636 East 14th Street at Avenue Z,  presents an afternoon of outdoor music and hot chocolate celebrating the Brooklyn Public Library's 125th Anniversary on Saturday, April 2, 2020, 3:00pm - 4:00pm. Middle School Admissions Virtual Events for Families

NYC Schools Account

NYC Schools Account

The NYC Department of Education is requesting that all NYC Public School students have a NYC Schools Account.  In this account, you will be able to view your child's state exam scores, attendance, grades, reading level, Fitnessgram results, report cards, and much more.  This year you will be able to nominate yourself to be elected to the District 21 Community Education Council, and vote for others to be elected, through this account.

How to create a NYC Schools Account

You will need the Account Creation Code that in previous years has been sent home with your child if they attended PS 128 prior to Sept. 2020, and most recently mailed in October 2020, to all PS 128 families who at that time did not have a NYC Schools Account.
 You will need this code and your child’s 9-digit student ID number to create an NYC Schools Account or to add additional children to an existing account.
1. Visit and select the “Create account” link on the login page.
2. Enter your email address and create a new password.
3. Upon signing in for the first time, you will need to select and answer three security questions and
validate your email address.  

4. Enter your child’s 9-digitstudent ID number. You can find your child’s ID number on their report
card. If you do not have your child’s ID number, contact your child’s school.
5. Enter the unique Account Creation Code.

P-EBT Information

Because New York City is a universal school lunch district, all public school families regardless of income will receive the benefits! Familieswill soon receive up to $1,320 in food benefits to help cover the cost of missed school meals while students learned remotely during the COVID pandemic. You do not (and cannot) apply for Pandemic EBT food benefits. P-EBT food benefits will automatically be issued to eligible children. Read more about it on the NY State announcement ,

Kindergarten Application Available Wednesday, December 8, 2021

  Beginning Wednesday, December 8, 2021, children born in 2017 can apply to Kindergarten for the 2022-2023 school year. Families can apply to Kindergarten two ways, online at MySchools or over the phone by calling 718-935-2009. includes an online tool that allows families to explore program choices, save programs of interest and submit their application.. Below please find Elementary School Admissions Events in English, Arabic, Chinese, Russian, Spanish, and Urdu.  At each event you will be educated on 3-K, pre-K, and Kindergarten admissions, how to create your account, explore your school and program options, and get ready to apply.  After each presentation, there will be a question and answer segment.

P.S. 128 Celebrates 100th Day of School

Tomorrow, Wednesday, February 16, 2022, we will be celebrating the 100th Day of School.  Staff and students will celebrate by wearing something that represents 100.  Some examples are:

-put 100 stickers on a shirt

-make a necklace with 100 beads

-make a poster with 100 items on it

Use your creativity and let's have fun celebrating the 100th Day of School.

2022 NYC School Survey

Let your voice be heard! Please take a few minutes to complete the Learning Environment Survey that was sent home with your child/ren on Thursday, March 3, 2022.  This is a wonderful opportunity for you to inform us of your thoughts about P.S. 128.  We ask that you complete the survey for each child that attends P.S. 128 and return to your child's teacher in the green and white envelope that was provided with the survey.  If you choose to complete the survey online, please visit to do so.  Once completed please print out the confirmation sheet, label with your child's name and class and return to your child's teacher. The deadline to submit your survey is Friday, March 25, 2022. All students who return the survey to their teacher will receive a prize.

2022 NYC School Survey Update ( 3-14-22)

If you have submitted your 2022 NYC Schools Survey(s) thank you.  If you have not submitted your survey(s) please do so as soon as possible.  Please make sure to fill in the circle that corresponds with your answer with blue ink, black ink or pencil.  No other ink colors, checkmark or any other symbol will be accepted or counted as completed.  Those of you that have submitted your survey using any other ink colors, checkmarks or any other symbol, may retake your survey online at  Please print out the confirmation sheet, label it with your child's name and class, and send in to your child's teacher for your child to receive a prize.  The deadline to submit your survey(s) is Friday, March 25, 2022.

Great News! The 2022 NYC School Survey Deadline Extended to Friday, April 15, 2022

The deadline to complete and submit your child's 2022 NYC School Survey has been extended to Friday, April 15, 2022.  We are asking if you have not completed and submitted your survey to your child's teacher, please do so as soon as possible. You may also complete online. If you wish you may use the computer in my room, 214. Our school was selected to participate in Kinsa'a FLUency School Health Program sponsored by Lysol.  This program offers us the opportunity to give FREE Smart Thermometers to all school families and staff staff.  Ordering yours is  easy! Kindergarten Save for College Scholarship Activation Meeting

I would like to once again inform you that as a Kindergarten student, your child was automatically enrolled in the NYC Kids RISE Save for College Program which means your child now has an NYC Scholarship Account with an initial $100.00, and opportunities to earn more over time.  If you did not opt out of the program, you should have or will be receiving a Welcome Kit in the mail from NYC Kids Rise with more details. On Wednesday, May 18, 2022, we will host our Kindergarten Save for College Scholarship Activation Meeting at which time we will assist you with activating your account .  An invite was sent home with Kindergarten students.  Please take a minute to complete the tear-off and send in to your child's teacher no late than Friday, May 13, 2022.
