Middle School Admissions Process 2022-2023
The Middle School Admissions Process for all 5th grade students is scheduled to begin early Fall 2022.
Please know that this process consists of several steps in choosing your child's Middle School. All 5th grade students are to complete an online application, regardless of what school you would like your child to be considered for. When the application becomes available, families will receive a letter from the Department of Education with instructions for the first step. If I have access to these letters, I will send home a copy with your child. P.S.128, as well as the Office of Student Enrollment will host information sessions on the middle school admissions process. At this time it is not confirmed if they will be virtual, hybrid, or in-person. The dates and times are to be determined.
More information to come - stay tuned!

NYC Schools Account
NYC Schools Account
The NYC Department of Education is requesting that all NYC Public School students have a NYC Schools Account. In this account, you will be able to view your child's state exam scores, attendance, grades, reading level, Fitnessgram results, report cards, and much more. This year you will be able to nominate yourself to be elected to the District 21 Community Education Council, and vote for others to be elected, through this account.
How to create a NYC Schools Account
You will need the Account Creation Code that in previous years has been sent home with your child if they attended PS 128 prior to Sept. 2022 and mailed to homes in October 2020, to all PS 128 families who at that time did not have a NYC Schools Account.
You will need this code and your child’s 9-digit student ID number to create an NYC Schools Account or to add additional children to an existing account.
1. Visit https://mystudent.nyc/ and select the “Create account” link on the login page.
2. Enter your email address and create a new password.
3. Upon signing in for the first time, you will need to select and answer three security questions and
validate your email address.
4. Enter your child’s 9-digitstudent ID number. You can find your child’s ID number on their report
card. If you do not have your child’s ID number, please contact me or the main office.
5. Enter the unique Account Creation Code.
If you need assistance or have questions about navigating your account, contact me, Rose Ann Montemurro, Parent Coordinator, at 718-373-5900, 929-617-5167, rmontemurro@schools.nyc.gov, or call 311.
